Welcome

Welcome to St. Vincent de Paul Rehabilitation and Nursing Center in Berlin, New Hampshire, where person-centered care is our priority, and our motto is “One heart, one home, one family.”

Whether you are a prospective resident or family member, we understand you may experience a variety of emotions as you arrive at a skilled nursing facility like ours. That is why we are committed to an individualized experience of compassion and care. Our highly qualified staff is ready and eager to answer your questions, meet your healthcare needs and satisfy your personal requests. You will have your own interdisciplinary team working with you to create an individualized plan of care to ensure your needs, preferences and wishes are continually met.

Our unique approach to personalized support, healthcare needs and guest experience goes beyond rehabilitation and skilled nursing.

Nursing – 24/7 nursing care from experienced staff allows St. Vincent’s to deliver exceptional medical services and health monitoring. Our nursing staff has multiple certifications and we contract with various providers, allowing for a wide range of services to be conveniently available to residents, such as on-site physical, occupational and speech therapy services, IV therapy, podiatry services, dietitian services, dental services, wound care, and so much more.

Dietary – Knowing that everyone has varying dietary and nutrition requirements, we will work with you to ensure you are offered a variety of meal options to meet your needs. Residents have the opportunity for personalized dining preferences, including menu options and the time they eat. In addition to a rotating menu, items are specific to the area culture and change throughout the year. You will also have a chance to shape future menus, as residents and guests can taste test new items.

Spiritual Connections – We welcome all religious and spiritual beliefs at St. Vincent’s. We have a beautiful Catholic chapel that is accessible 24/7 for residents and guests.

Memory Care – Our Memory Care unit is a secured unit, providing additional safety and security for residents with memory impairments. This special unit has dementia-focused activities designed specifically to benefit individuals with dementia by providing purposeful activity, structured routines and a supportive social environment.

Beyond the highest level of passionate and individualized care, St. Vincent provides numerous opportunities for you to entertain guests or find solitude. Our home offers multiple spacious common and lounge areas to gather with family and friends. You are never too far away from a furry visit; St. Vincent is pet-friendly and welcomes vaccinated animals. During the summer months, you are welcome to enjoy the patio and Koi Pond in our outdoor patio space while taking in the serene view of the White Mountains. And you’ll maintain the level of privacy you won’t find many places – 98% of our rooms are private. Our aim is to provide every opportunity for you to find comfort and empowerment of care throughout your stay.

We recognize the difficult challenges that are presented when considering rehabilitation or long-term nursing care, and we are committed to being your community resource through your journey. We encourage you to review our website and schedule a visit to experience for yourself what it means when we say, “One heart, one home, one family.”
