Long-Term Care

Long-Term Care and 24/7 Nursing

St. Vincent Rehabilitation & Nursing Center provides long-term care and 24/7 nursing support for individuals seeking a community where you receive the highest level of care – and experience the highest quality of life.

Since 1963, we have proudly served the needs of residents across the North Country through our person-centered model of care. This means that your interests, goals, wishes and expectations become the foundation of your own personal care plan. You can live life on your own terms while finding the optimal balance between physical, emotional, social and spiritual wellness.

Is Long-Term Care Right For You?

Leaving your home for long-term care can understandably be a hard process full of emotions. You might worry you’ll lose your independence, miss your familiar surroundings and no longer be able to do your favorite things. For your family, there might be angst on how you will respond to a new setting and interact with the people and environment around you.

But in many cases, long-term care is the best way to meet your increasing daily needs – across all facets of life. It’s something we’re fully confident you’ll realize here at St. Vincent.

Today’s skilled nursing facilities are held to higher quality standards and have higher levels of nurse staffing. They can also be a better value with a single, all-inclusive cost per day. There are also more services and amenities to give you the true feeling of being home.

Why St. Vincent?

St. Vincent is a place our residents are proud to call home, as reflected in our 97 percent resident satisfaction levels, which are well above industry standards.

98 percent of our rooms are private. You likely won’t find that much anywhere else, giving you the privacy, safety and security you deserve.

Our emphasis on person-centered means the support you receive is solely based on your own unique needs and lifestyle.

We first sit down with you and your family members and get to know everything about you. We go over your medical and life history, routines, hobbies, goals, expectations, limitations, dietary restrictions and so much more. We understand what you like (and what you don’t like), what motivates you, and what makes you feel most safe and comfortable.

Everyone is different, and that’s why we come up with an individual care plan that is essential in creating an environment where you not only positively engage more in life – in both new and old ways  – but one you embrace and thrive within. The way we see it, you direct your own care.

Your safety here at St. Vincent is paramount too, with 24/7 care and support. The nursing staff is never more than a few feet away and our electronic call system ensures we always respond to anything you need in the most timely and efficient manner.

It is no surprise that many of our residents find that living at St. Vincent is a preferred alternative to living alone or with family members.

When you enter the front door at St. Vincent, you become family – and part of a community that opens up many new opportunities for you.

Step into our lobby and you’ll meet staff ready to support your every need. Nearby is the activities room, where you’ll find endless enrichment, including regular entertainment, games, music and so much more.

Walk down into our residences and you’ll find our 70 spacious resident rooms, 69 of which are private and all a short distance away from a nurse’s station. Our rooms are spread across two floors, with an elevator providing easy access up and down.

We want your room to feel like home, which is why we encourage you to bring your favorite pillows, clothing, electronics, personal care items or even your own furniture. Your walls await decoration, whether it’s precious family photos or your favorite artwork. Your room comes complete with access to Wi-Fi, phone and cable television, so you can continually stay connected with loved ones or simply enjoy your favorite television program. Visitors are always welcome.

There is an abundance of personalized dining options to meet your needs, whether it’s enjoying delicious cuisine in our neighborhood dining areas or restaurant-style dining in our main dining room.

If you seek time to reflect outside, spend time with family or just catch some fresh air, we offer you plenty of outdoor space. Our residents especially love the garden area and koi pond. The view of the White Mountains is breathtaking.

While you receive 24/7 nursing care, we ensure you’re happy, engaged and feel right at home. After all, here at St. Vincent, you’re family.

Resident safety, personalized attention and compassionate care is the foundation of high-quality healthcare at St. Vincent.

We are recipients of the 2018 & 2019 Eli Pick Facility Leadership Award and the 2019 AHCA National Bronze Award for Commitment to Quality (2019), both national honors for health, safety and quality excellence. We’re also proud of our 97 percent resident satisfaction level, which is well above industry standards.
